DESCRIPTION: This is a report of the testing of backflow devices, installed in water systems cross connections, to ensure that pollutants do not contaminate drinking water. It includes the name of a water system, the location of the backflow device, the name and address of the owner of the device, the size, model number and serial number of the device and the name of the manufacturer, the results of the initial test of the check valves, differential pressure relief valve, and pressure vacuum breaker, any repairs to them, and the results of the final test, the name and certified tester number of the tester, the date of the test, the name of the individual conducting the repairs, the date of the repairs, the name and certified tester number of the final tester, the date of the test, whether the test performance of the device was satisfactory or unsatisfactory, the signature of the device owner's representative, the number of the water system, and the device number.
