DESCRIPTION: These are duplicate copies of EEG Tracings made as part of a complete physical examination of participants in the Handicapped Children's Services program. The records include the EEG chart, EEG Requisition, and occasionally a narrative report of the test results, the child's name, sex, address, and age; the date of the test; the test conditions (if patient awake or asleep); the type and amount of medication given with the test; the patient's chart number; and a provisional diagnosis.
