DESCRIPTION: This is a log of cases being reviewed by the Utilization Management Unit of the bureau to ensure that all reviews mandated under 42 CFR 456 are done in a timely manner. These records include the log number, the name and identification number of the health care provider, the source that caused the review to open, the date(s) the case was reviewed by the Utilization Control Committee, the activity taken on that date(s), the name of the reviewer, the date the case was closed or referred for further action, the date the provider reimbursed the state for an erroneous payment, and the date any adjustment to the provider's account was made.

Destroy.
