TITLE: Inspection of care reports

DESCRIPTION: These records contain on-site surveys of skilled nursing facilities, intermediate care facilities, and institutions for the mentally retarded. These surveys are conducted by the Medical Review Bureau to ensure that such facilities are complying with state and federal standards. These reviews are required by 42 Code of Federal Regulations 431.610(g)(3).

Included in this record are Inpatient Nursing Medical Care Skilled/Intermediate Care Report, Intermediate Care for the Mentally Retarded Report, Inspection of Care Findings and Plan of Correction, Problem Identification Sheet for Inspection of Care, Inspection of Care Revisit Report, Inspection of Care Summary of Findings Not Corrected, Inspection of Care Summary Report, and Inspection of Care Referral/Response form. Information within these reports and forms include: a list of patients certified for treatment with name, identification number, date of birth, facility location, admission date, transaction control number, and diagnosis; an inspection of specific patients including level of care, facility name and identification number, services, and medical records; a statement of findings including deficiencies and provider's plan of action to correct said deficiencies; and date(s) of the inspection.

Paper: Retain in Office for 2 years and then transfer to State Records Center. Retain in State Records Center for 3 years and then destroy.

This record has administrative value. Records such as nursing home complaints are retained for a period of 5 years. Retaining this record for that length of time would be consistent with similar records' retentions. A change in classification is being made to meet the requirements of 42 Code of Federal Regulations 431.115.
