DESCRIPTION: This is computer output microfiche number 68210M, part of the Surveillance and Utilization Review Subsystem. This is used by the department to evaluate the norms set up for each treatment and diagnosis class in order to find exceptions to the norm. These records include the report date, the diagnosis and treatment class, the reporting periods (up to five) covered by the report, the numbers that are considered average, standard deviation, the lower limit, and the upper limit, and the relative weight given to each, the totals number of profiles process, and the totals printed and non-printed for each category.
