DESCRIPTION: These records are used to file an insurance claim and to ensure that the state has collected on the claim. After payment, the record is used for reference. The agency gets a copy and public safety will have a record of the accident. Risk Management will also get a copy of this record. If any litigation results Risk Management will handle it.

The information includes date of accident, location, people involved, description of the accident, and signature of the driver.
