DESCRIPTION: Tool used by the agency to determine the varying needs of the clien and to determine appropriate service provisions. This is a primary component of service coordination. All activities are directed towards the development and monitoring of need services through the individuals plan. The progress is evaluated and new goals and objectives are set. This series includes long term gouals, measurable objectives, methods, start and target date, rewards, coalition plan, Individual Education Plan, Review Of Service Plan, the areas to be assessed, graphs and team meeting notes.
