DESCRIPTION: This is a report completed by the auditor which identifies potential safety hazards hopefully before an accident or personnel injury occurs. Includes date, store number, address, individual conducting the inspection, description of any problems found, and person responsible for making the correction. Areas inspected are: general premises; flammable liquid storage area; electrical equipment; smoking policies and procedures, fire exits, fire extinguishers, automatic sprinklers, sprinkler control valves, other items noted, inspected by, phone number of inspector. The records in this record series collect the following personal identifying information: First & last name combined with a phone number and business address. This information is collected in order to notate the inspector, the area inspected, those responsible and is not used for any other purpose.
