TITLE: Injury compensation payroll case files
DATES: 1950-2003.
ARRANGEMENT: Chronological by year and therein by case number.

DESCRIPTION: These files document individuals on payroll after being awarded compensation for an industrial accident. They document those individuals receiving permanent total compensation, permanent partial compensation, and those receiving temporary compensation if payments for such are being made over a period of time. Temporary payment files will be refiled under Final Payment Case Files (see series 80723) at time of final payment. The files include a description of the injury and how it occurred, medical reports, a request for hearing, hearing notices, summaries of the hearing, compensation agreements, an order of payment from the Industrial Commission, affidavits, administrator's notes from the hearing, the amount awarded, and the date(s) of payment.
