DESCRIPTION: These records support the agency's function to document and certify the facts of births and family formation. These records document the parentage of a child who is legitimized by the marriage of his natural parents as determined by the agency, or whose parentage has been determined by any court having jurisdiction (Utah Code 26-2-10(2008)). Information includes the parent's identifying information, and the child's original and supplementary birth certificates. Information may also include the affidavit for voluntary declaration of paternity, the court determination of paternity, or a copy of the parents' marriage certificate. Index is included.
