DESCRIPTION: This is a report submitted by hospitals of live births that occurred during the month. This is used by the bureau to ensure that all birth certificates were received as required by UCA 26-2-5. The information includes the month of the report, the name of the hospital, and a list of births giving the date of birth, the name of parents, the child's sex, and the name of the medical attendant.
