DESCRIPTION: This records the actions of the State Health Department Certified Food Service Evaluation Officers for the training of local food service personnel in the uniform use and interpretation of sanitation requirements and related preparation of the inspection report forms. The purpose of the training is to assure uniform inspection competency throughout the state. These files includes Local Food Service Evaluation and Inspection Officer Background Data, Utah State Health Department Procedure for Standardization of Local Health Department Food Service Inspection Officers, and Food Service Establishment Inspection Report. Information includes the inspection officer's name, job title, office address, home address, length of service with the agency, present duties, educational qualifications, experience in food service sanitation, signature, and date of previous standardization; the results of joint inspections with the State Certified Evaluator, including the types and numbers of violations and the percent of violations discovered by the local officer; and the results of the individual inspections.

FORMAT MANAGEMENT

Paper: Retain in Office until the Inspection Officer's certification expires or until it is terminated and then transfer to State Records Center. Retain in State Records Center for 2 years and then destroy.

As long as the inspection officer is certified, information as to his/her qualifications and the record of joint inspection should be maintained. Once the certification has expired, this information may be useful as background material should the officer wish to be restandardized. Should the inspection officer certification be withdrawn due to inadequate performance, a record of the joint inspection would help defend the state should the inspection officer wish to dispute the withdrawal. A two year retention was chosen because it is the period of time within which a discrimination action must be taken. This activity is part of the state's responsibility under UCA 26-24-3.
