DESCRIPTION: This is a list of dentists who have been contacted and given information on the state's sealant program. This program was initiated in 1982 as a community education program to contact dentists and to discuss with them the benefits and cost effectiveness of sealants in dental treatment. The log includes the health district, the dentist's name, and the date of contact by mailing information, leaving information at the office, or personally visiting the dentist.
