DESCRIPTION: This is a file of referrals from the Social Security Administration to the bureau of children who are eligible for services. The bureau takes the referral and sends an application form to the family involved. This file is maintained as a suspense file until the application is returned. It includes the name and address of child and responsible adult, the child's social security number and date of birth, the child's sex, the parents' marital status, the family's income level, parent's employment, a description of the amount and type of other assistance, the previous year's medical expenses, and any insurance coverage.
