DESCRIPTION: This is a record of information and data collected on patients currently receiving treatment in the sex offender program. This information is used to study how effective the treatment is over a period of time, to support administrative needs, for statistical and program monitoring reports, and to analyze and redevelop the current program.
This series includes Patient Admission Data, date the program was completed, charges brought against the patient, the name of the probation officer, Beck Inventory Questionnaire, follow-up questionnaire, progress notes, laboratory findings, and self evaluations.
