TITLE: Patient visitation request slips

DESCRIPTION: This is a form that must be completed by the visitor when they arrive at the hospital. This slip gives the visitor access to the unit where the patient resides and gives the hospital a record of who visited each patient, as well as when and how often they visited. The records in this series include the date of visit , visitor's relationship to the patient, visitor's address and telephone number, and the patient's name and unit.
