DESCRIPTION: This series contains records of care provided to patients by the optometrist, including eye examinations, prescriptions for corrective lenses, and general eye care. These records are also used to compare previous examinations with current examinations to monitor and treat changes in vision or conditions as a result of medication. Information in the records includes date of visit, list of medications being taken by the patient, precautions, findings, and signatures.
