DESCRIPTION: These are reports from the local health districts as to their activities in the Well Child Conference program. It is used by the staff to monitor the activities of the clinics and of the program as a whole. It includes the report period, the clinic location, and by ethnic background and age groups, the number of re-admission cases, first visits, re-visits, number examined by a doctor, the number examined by a nurse, the number referred for care, the number below poverty level, the number referred for counseling, and the number of DPT, polio, MMR, and Td immunizations.
