DESCRIPTION: This is a record which lists the patients and what Community Health Center they were admitted from (Cache, Salt Lake). The listing is sent to the Community Health Center administrator, and is used when they need to check up on a patient, see if they are still in residence, or have been discharged. This record is also used by administration to see what the patient/space status ratio is.

This series includes the ward, dates of admission and discharge, the name of the Community Health Center, and commitment status (voluntary, court).
