DESCRIPTION: These report forms are completed for each fire and are forwarded to the State Fire Marshall. These forms include Fire Department indentification number; incident number, date, alarm time; arrival time; type of situation found; type of action taken; cause of fire; address; name of occupant; name of owner; telephone number; type of vehicles and crews responding; incident related injuries or death information; area of fire origin; form of heat or ignition; form and type of material ignited; method of extinguishment; estimated dollar loss; extent of damage; detector or sprinkler performance; smoke information; whether mobile property or equipment involved; name and position of officer on charge; and name and position of employee completing report. This information is stored on hard disk and backed up monthly and yearly on tape.
