DESCRIPTION: These records contain patient information on reportable communicable diseases. As stated in the Utah Code of Communicable Disease Rules, the Department of Health collects this information in order to promote and protect the public health and to prevent and control disease and epidemic infections. Information includes patient's name, age, sex, race, address; name of disease, date reported, and date of onset; laboratory data; report by number of cases only; physician's name, address, and telephone number; and information on venereal disease, tuberculosis and viral hepatitis, if appropriate.
