TITLE: Breathalyzer affidavit

DESCRIPTION: The breathalyzer affidavit gives certification and proof that breathalyzer instruments have been tested and utilized by the Utah Highway Patrol. Information includes serial number and location of equipment, date of inspection, a complete listing of all tests performed on the equipment (such as if it is heating properly, providing correct air samples, if the null meter and photo cells function properly, etc.). The inspection notes any required repairs. Also included are the names and signatures of technicians performing tests, and signature of the notary public.
