DESCRIPTION: This file documents the proceedings of an unemployment insurance claimant/employer lower authority appeal. It contains all forms and paperwork compiled in the history of the claim to date such as initial claim forms, claimant/employer statements, agency personnel statements, adjudication decisions, eligibility review decisions, etc. as well as the notice of hearing and the appeal decision. This file not only documents the history of the claim and the hearing decision, but it is also used as the base for any subsequent hearings such as Board of Review, Court of Appeals, and/or Supreme Court.
