DESCRIPTION: These are bills received from Yellow Cab company for transporting clients in the Social Services Health Plan to clinics or health care providers. Transportation is required by 42 CFR 431.53. These documents include the vendor's number, name, and address, the amount of the billing, the date of transportation, the name of the client, the client's social security number, and the addresses where the client was met and taken.
