TITLE: Death certificates

DESCRIPTION: This series contains the registrar's copy of death certificates issued and signed by physicians at the State Hospital. The official state copy is filed with Vital Statistics (series 81448).

After 1981 responsibility for issuing death certificates transferred to the State Medical Examiner and copies are filed in the patient's medical record. The certificates include patient's name, physician's signature, cause of death, date of death and time of death.
