DESCRIPTION: These are permits for private companies to work on city owned property ("public way") for demolitions, excavations, blasting, crane operation, barricade installation, concrete construction (curb, gutter, or sidewalk), or the moving of heavy equipment. Includes: job address, date, name of owner/agent at job address, name of applicant, phone number of applicant, work order number, mailing address, contractor, contractor's phone number, state license number, state license certification, diagram of work required, checklist of existing utilities and construction, traffic control plan, approximate starting and completion dates, verification of insurance, bonds, and license. Also includes department information such as fee amounts, inspection information, field measurements, acceptance, and approval. signatures.
