DESCRIPTION: These case files are created by foster care social workers and are used to document client needs and problems, to track progress and to record services provided by Human Services, Family Services Division. Information contained in these files include payments, check requests, client characteristics (Form 1054), eligibility documents such as placement agreements, applications of verification of income, check stubs, eligibility determination, immigration and naturalization forms, decision notices, long-term foster care agreements, prior approval and recertifications, child care agreements, social security cards, and birth certifications. In addition, court documents include adoption forms 356, 356A, and 398, affidavits for taking children into custody, dispositions, reviews, and temporary court orders, copies of petitions, detention orders, initial court orders with prevention and juvenile court referral forms, permanent deprivation orders, shelter orders, orders to show cause, and summons. Assessments, correspondence, and collateral agency reports are also included in these case files.
