The form includes space to record the applicant's name, the date of the affidavit, place of birth, place and date of filing declaration of intention, date of arrival in the United States, the foreign ruler to which the applicant renounces allegiance, and the applicant's signature. The two standard witness affidavits provide space for such information as the witness' name, place and length of residence, length of aquaintance with the applicant, and witness' signature.
