These were to be filed at least two years before an individual could apply for citizenship. The first volume contains preprinted forms with blanks for the insertion of the individual's name\, sovereign\, date\, and signatures of the individual and court clerks. Beginning in 1906\, more detailed federal forms were used. These also included personal description\, birthplace and date\, residence\, and emigration and immigration data. By 1916\, space was included to record marital status\, spouse's name\, birthplace and date.__By the 1930s more space was added for sex\, race\, nationality\, marriage date and place\, and the number of children with their,birth date\, birthplace\, and residence; any previous declaration of intention; and his or her name at entry. In 1941 information was added regarding any departures from the United States.
